OUR SERVICES

Our services include Life Skills training to support self-sufficiency and independent living; Educational advocacy and support, tutoring and college prep; Case management, referral and coordination for health services; Crisis intervention; Activities of daily living; Individual and group therapy; building strength of personal identity providing self-esteem and confidence.

 

Residents are referred to the Group Home through the Department of Jobs Family Services (DJFS), Unified Case Management, Care Management Organization, and Youth Case Management. Girls may also be identified as needing this service by school, mental health centers, social service agencies, the courts, and private practitioners.

 

A referral packet should contain the following (if applicable):

  • Social History
  • Mental Health Evaluation (if applicable)
  • Educational Evaluation and IEP (if applicable)
  • Complete Health History
  • In the custody of State of Ohio
  • Appropriate for public school attendance
  • Willingness to participate in program

 

Once adequate information is received the treatment team will make a decision. If deemed appropriate, and if there is an opening, an interview with the Executive Director is then scheduled.
